Requisitions for In-Office Draws

Follow the steps to place a lab order #

Place an order with labs (for details visit: https://www.metaboliccode.com/knowledge-base/ordering-assessments-with-labs-without-insurance/)

  1. Choose a Metabolic Code® Panel and any additional lab markers you would like to order.
  2. Select the Checkbox next to Blood Drawn In Office.
  3. Click the Calendar icon to select the date and time of the anticipated collection. Keep in mind that you will still need to hand-write the time and date of collection as below.
  4. After submitting the order, download and then print all pages of the requisition and enough copies to include in each specimen bag.
  5. Hand write both the Date of Collection (DOC) and Time of Collection (TOC) on each page and on each copy of the requisition:
    • Make sure you print a copy for each specimen bag or else the lab will not run the tests listed
    • Insert a copy of the requisition into each specimen bag for pickup.
